Decorah School District reminds athletes to get an athletic physical each year

The Decorah School District is reminding parents that any student interested in participating in athletics at Decorah High School or Decorah Middle School must have an updated student athletic physical every 12 months.
 
These physicals (otherwise known by insurance companies as "Well Child Checks") should be scheduled at a healthcare provider to fulfill the athletic requirements for the following school year.  Many insurance plans cover one Well Child Check per year at 100 percent reimbursement.  Please check with your insurance company to make sure you understand your coverage before the appointment.  If you do not have insurance or have already fulfilled the Well Child Benefit on your plan this year, the local clinics provide discounted athletic physicals.
 
All student-athletes planning to participate in a fall sport are urged to complete their Well Child Checks/physicals prior to August 1st.  (Especially those entering the 7th Grade.)  By completing these appointments prior to August 1st, students will be more prepared by having their physicals and physical dates eligible to be completed prior to the first dates of practice for their fall and winter activities for the remainder of their years in middle school and high school.
 
The physical form to bring to the Well Child Check can be downloaded at http://www.decorah.k12.ia.us/UserFiles/Servers/Server_3164137/File/Decorah%20HS/Physical%20Form%202010.pdf.  Providers ask that students please have their physical form when they come for their appointment.  Students under the age 18 must be accompanied by a parent or have a signed parental consent form.
